چکیده

Abstract Background The optimal treatment in patients with gastric cancer and peritoneal disease remains controversial. Some guidelines indicate palliative only, while others consider surgical case of positive lavage cytology (CY+) or limited disease. Here, we analyzed the role cancer, prognostic relevance response to neoadjuvant therapy. Methods In this retrospective cohort analysis, adenocarcinoma stomach esophago-gastric junction from a single center operated between 2011 2019. According histology cytology, were classified into four risk groups: (A) no disease, (B) CY+ who converted negative (CY?) after chemotherapy, (C) without conversion (D) visible metastasis. Results Overall, n = 172 included. At initial presentation, 125 (73%) had about third ( 47, 27%) microscopic macroscopic Among them, 14 (8%) metastasis, 9 CY? 5 was observed. Median overall survival not reached initially resulting 3-year rates 65% 53%. contrast, median reduced 13 months (95% CI 8.7–16.7) 16 12–20.5) metastasis difference two groups p .364). rate significantly higher FLOT (5-fluorouracil plus leucovorin, oxaliplatin, docetaxel) compared ECF (epirubicin, cisplatin, 5-fluorouracil) 0.027). Conclusion Conversion chemotherapy is significant factor for better survival. Surgical well-selected should therefore be considered. However, recurrence frequent despite conversion, urging local control.

Prospective clinical trial of diagnostic peritoneal lavage to detect positive peritoneal cytology in patients with gastric cancer.

BACKGROUND AND OBJECTIVES Positive peritoneal cytology equates M1 disease in patients with gastric cancer. Diagnostic peritoneal lavage (DPL) is a proven test to detect occult visceral injury in trauma patients. The objective of this study is to determine whether DPL can be used to assess peritoneal cytology in patients with gastric cancer. METHODS Patients with gastric adenocarcinoma were pr...

Prognostic role of positive peritoneal cytology in patients with resectable gastric cancer.

OBJECTIVE To evaluate the prognostic value of positive peritoneal lavage in patients with gastric cancer without signs of peritoneal or hematogenous spread. METHODS We evaluated patients with gastric adenocarcinoma treated with curative intent operation. The peritoneal lavage was classified as positive or negative for neoplastic cells. We obtained demographics, performance status, histology a...

Positive peritoneal lavage cytology is a predictor of worse survival in locally advanced pancreatic cancer.

BACKGROUND Peritoneal lavage cytology in the staging of pancreatic cancer is not widely used given improvements in computed tomography (CT). The aim of this study was to determine the utility of peritoneal lavage cytology in predicting survival in locally advanced pancreatic cancer. METHODS Between 2000 and 2008, 202 patients with biopsy-proven pancreatic cancer who were determined by pancrea...

Incidence of positive peritoneal cytology in patients with endometrial carcinoma after hysteroscopy vs. dilatation and curettage

BACKGROUND The aim of the study was to compare the frequency of positive peritoneal washings in endometrial cancer patients after either hysteroscopy (HSC) or dilatation and curettage (D&C). PATIENTS AND METHODS We performed a retrospective analysis of 227 patients who underwent either HSC (N = 144) or D&C (N = 83) and were diagnosed with endometrial carcinoma at the University Medical Centre...

Assessment of Prognostic Value of Peritoneal Lavage Cytology in Patients with Resectable Gastric Cancer

There are wide differences in the gastric cancer occurrence among different geographical regions. In Iran, the northern and the northwestern regions are with the most higher risk of gastric cancer occurrences. In spite of the recent gastroscopical examinat...
